Completed forms can be submitted by handing them to any SCPO staff member, mailing to SCPO, 30 Millbank St, Rochester, NY 14619, or emailing to emartinez@scporoc.org.

Resident & Alumni Exit Survey
Anonymous survey for Jennifer House, Nielsen House, and Permanent Supportive Housing residents and alumni. Every question is optional and takes about 10–20 minutes.
